Holistic Primary Care Perspective
IHMC's care model is built around looking at the whole patient. Primary care visits may include attention to chronic conditions, medications, mobility, skin concerns, foot concerns, circulation concerns, safety risks, home support, and care coordination needs. When a concern requires additional support, IHMC may coordinate referrals, home health orders, specialist care, or other clinically appropriate next steps.
