Submitting a Referral

To refer a patient to IHMC, please contact us by phone or through the contact form. We will review the referral, confirm service area eligibility, and follow up with you and the patient or caregiver to begin the intake process.

When submitting a referral, it is helpful to have the following information available:

  • Patient name and date of birth
  • Patient address and contact information
  • Reason for referral and brief clinical summary
  • Insurance information or self-pay status
  • Referring provider name and contact information
  • Any relevant medical records or clinical notes
