Background
Keshona Isaacksz is a board-certified family nurse practitioner, Certified Wound Specialist, former Medical Director, and the Founder and Clinical Director of Inherited Health Medical Center. Her background spans primary care, chronic disease management, mobile healthcare, wound care, foot care, medically complex patients, clinical operations, and clinical leadership.
She founded IHMC to provide more personal, accessible, and coordinated primary care for adults — a practice built around understanding the whole patient, not just the presenting complaint.
Clinical Experience
Her clinical experience in wound care and foot care strengthens her ability to look at the whole patient, identify risks early, educate patients and families, coordinate care, and make appropriate referrals when needed. These skills inform a primary care approach that is thorough, attentive, and clinically grounded.
Her background in mobile healthcare and working with medically complex patients gives her a practical understanding of the barriers that affect health, safety, and follow-through — and how to help patients and families navigate them.
Care Philosophy
Keshona's care style is attentive, educational, practical, compassionate, and collaborative. She believes patients and families deserve clear explanations, realistic care plans, and a provider who considers the full picture — not just one symptom or one visit.
She is committed to careful assessment, clear communication, practical care planning, and clinically appropriate next steps. She does not guarantee outcomes, but she is committed to showing up prepared, listening carefully, and following through.
